MEDICAL INFORMATION:

I understand that it is the student/parent/guardian of students responsibility to disclose all relevant medical conditions, limitations, disabilities, ailments or impairments at time of enrolment. Otherwise, all students will be deemed as clear from all medical conditions, injuries, limitations and disabilities

I understand that it is the student/parent/guardian of students responsibility to ensure a copy of any Asthma or Anaphylaxis plans are sent through to riverinadancecapital@gmail.com and received at time of enrolment. It is also the student/ parent/guardian of students responsibility to ensure all necessary puffers, preventers, epi pens etc are with the student and labelled anytime they are at the studio or event associated with the RDC - as well as a physical plan to accompany them. It is the responsibility of the student/parent/guardian to give these to an RDC teacher or representative at the start of class.

LIABILITY WAIVER I am fully aware of my own/child’s health and physical limitations and understand the activity I/my child is participating in has the potential to cause injury and harm I therefore acknowledge my/my child’s participation is at my/their own risk. I therefore agree that it is the responsibility of myself/my child to participate within my/their own limitations and capabilities. It will be assumed by all representatives and/or teachers at RDC that I/my child is fit and healthy to participate unless informed otherwise. If informed otherwise I understand participation is at my/my child’s own risk and release.

With that knowledge, I therefore permit the release of RDC, RDC representatives and RDC teachers from all liability for accidental injury, illness or demise that may result from my/my child’s participation in any classes, workshops, competitions, exams and performances associated with RDC. I therefore assume all risks associated with participating in the classes, performances and workshops and consent participation.

It is understood and agreed that RDC will not be responsible for any loss of valuables, personal effects or clothing.

PHOTOGRAPHY / FILMING / PUBLICITY WAIVER

There may be times when the studio takes photography for advertising or educational purposes. If you DO NOT wish for you/your child to be photographed/filmed, please specify in writing to riverinadancecapital@gmail.com at time of enrolment. If no written advice is received, it will be assumed that you have given permission for your child to be photographed/filmed for this purpose. If no written advice is given, I, therefore give permission for representatives or teachers from RDC to photograph, film and or otherwise record my/my child’s performance in rehearsals, classes, workshops, concerts, demonstrations and any event associated with RDC. I too agree that these images and/or footage may be used for promotional purposes via social media, website, flyers, brochures etc.

There is to be no photography or filming of any RDC class, performance, workshop or event unless done so/permitted by a RDC representative. This is in adherence to our child safety policy.

There is to be no use of mobile phones within the studio, waiting areas or bathrooms/change rooms.

CHILD SAFETY

RDC is committed to the safety and protection of all children in our care. To ensure all children feel safe, happy and valued we have undertaken Child Safe Training with Keep Kids Safe in Dance and service NSW. We therefore have a statement of commitment to child safety which all staff and members of RDC must adhere to. By signing this enrolment form you agree to abide by and respect all child safe policies implemented by RDC.

PICK UP and DROP OFF

We kindly ask you to adhere to the following as terms of enrolment:

* please accompany your child to and from the studio door, we cannot have children running through car parks or across roads. Pick up and drop off must be to the door and by foot

* Please pick up and drop off on time.

* Students are supervised in the studio during class time (we cannot be responsible for supervision of students in between or before and after classes in the waiting area, toilets or change rooms). If you are running late for pick up, your child will wait inside the studio classroom for you to collect. We kindly ask this to only be in emergency cases.

* If you request for your child to wait in the waiting area in between classes or drop off early, it is understood that you are aware that no supervision is given in the waiting area in between classes and that RDC takes no responsibility.

* If there leaves only one student left on site with an RDC teacher when you collect your child; we kindly ask that you wait with this teacher and child until the other parent/guardian comes to collect- adhering to child safe policies.

EMAIL AMENDMENT (26/4/25) PARKING, PICK UP AND DROP OFF- STUDENTS MUST BE PICKED UP AND DROPPED OFF TO THE STUDIO DOOR ON FOOT

As with our previous locations, RDC has a very strict pick up and drop off policy. We understand this can be frustrating/inconvenient, but we do so for the safety and wellbeing of our students and their families. There is ample parking onsite and surrounding the studio; however, there is an inability to comfortably U – TURN in the car park if cars park or rest in NO PARKING AREAS. As with our Fitzmaurice and Chaston St locations, this will mean all students under 18 will require a parent to escort them into and out of the studio. No student will be allowed to leave the studio gates unless accompanied by a parent on foot. No students will be

allowed to run across the carpark to parked or waiting cars. There are to be no cars waiting in areas of the carpark that are not in an allocated parking bay.

We have a gated waiting area to avoid parents and students waiting in the car park, so kindly ask that you wait in there so not to crowd the car park. If your child is under the age of 18 and drives themselves to dance or is required to leave the studio without you. Please send us an email to riverinadancecapital@gmail.com indicating that you give them permission to leave the studio without the collection of a parent/guardian on foot.

We do also share our carpark with neighbouring businesses and residents. It is really important that you do not use carparks allocated to these businesses and residents.

*This clause will be amended on enrolment T and Cs. It will be assumed that all RDC students and families agree to this.

With this knowledge I therefore release RDC of all liability for accident, theft, injury or demise that may occur in the carpark and understand it is a condition of attending classes at RDC.

CHANGE ROOM/TOILET FACILITIES AMENDED 26/4/25

Change rooms and toilet facilities are to be strictly used by RDC students, teachers and guardians/parents only. By signing the below, you understand that no direct supervision is given in these areas and that in using these permit the release of RDC, any representative or teacher of RDC from all liability. For this reason, we encourage one parent to be on site in the waiting area for all of our under 5 classes in case they need to accompany students to the bathroom. This is only relevant to classes with one allocated teacher- some of our classes have multiple assistants to ensure a safe student to teacher ratio. Please email us if you are unsure.

The shower facilities will not be in use or permitted access. There is to be no use of phones or recording devices in change room/toilet facilities. Any property left in these areas will be unattended and It is understood and agreed that RDC will not be responsible for any loss of valuables, personal effects or clothing.

WAITING AREAS

The studio has two allocated waiting areas for students before class. Our existing policies remain as per enrolment terms and conditions for students and parents waiting . Our waiting areas are not as separated from the studio floor as Fitzmaurice street; so we kindly ask that all parents, guardians and students be respectful of classes in progress, with the noise in waiting areas being kept to a minimum. We also ask that only one parent/guardian per student be on site with their child if required. We are so lucky to now be opposite Turvey Tops shopping centre so hope families and friends can enjoy these facilities while students are in class.

With this said you understand that no supervision is given in waiting areas and these are to be used by students/parents/guardians at their own risk. RDC is therefore released from all liability for accident, theft, injury or demise that may occur in these areas.

CLASS EXPECTATIONS

All students are expected to arrive on time and be courteous and well behaved.

There is zero tolerance for bullying within classes.

No mobile phones or computers may be used during class time. There is to be no photography or video within the studio unless done so by an RDC representative.

Please note: all other staff members are required to pass on these questions on your behalf.

Parents/guardians are not allowed to interrupt a class in session. If you would like to enter the class at any time, please knock and wait quietly outside the door until indicated. This is not only for child protection purposes but also to reduce distraction. Dance with Me classes are the only exception to this rule.

Only water is allowed in the studio. No food, drinks, or gum. (Snacks in the waiting area are fine but please no nuts or seafood)

Correct dance attire and footwear is to be worn in class. No Jewellery to be worn in class and performances.

Consistent attendance in class is important, particularly in the lead up to exams, competition and performances.

TERMS OF ENROLMENT

Enrolment is compulsory for all students participating in classes @ RDC. Re enrolment is $40 and New Enrolment fee is $50 (which includes a studio bag)

Tuition Fees: Must be paid within 14 days of invoice. No refund will be given for lack of attendance or change of mind; however, a makeup class can be requested. Delayed payments and unpaid fees place enormous strain on our small business. If you are experiencing financial hardship, please discuss with RDC for a payment plan. If payment plans are not organised and/or adhered to within these 14 days a $20 late fee will be applied per fortnight if it is overdue.

All private lessons will incur a 25% cancellation fee - if informed under 24hrs.

I understand and accept that due to the nature of dance training, some physical contact may be necessary by the teacher to demonstrate exercises or technique during class or if there is a need to administer first aid. This is in accordance to our child safe politics.

I understand and agree to all outlined in student pick up, drop off and carpark policies I understand and agree to all outlined in the use of change room and toilet facilities.
I understand and agree to all outlined in the use of waiting areas.

I understand and agree to all outlined in the above liability and photography/video/publicity waiver.

I understand and agree that there is to be no photography or filming of any RDC class, performance, workshop or event unless done so/or permitted by a RDC representative.

I understand and agree RDC and all RDC representatives will not be held liable for any injuries / illnesses/ demise that occurs from participation in any class, performance, workshop, competition or event held by RDC.

I understand and agree to all RDC Childsafe policies.

I understand and agree that choreography used in class is under Copyright and not to be distributed or used outside of RDC.
